New York Compensation Law requires an injured worker (or person entitled to a death benefit) to be provided with the option to have the compensation payment made through direct deposit. It also requires them to be given notice of such option, and the forms necessary to enroll.
This Notice and Election Form for Direct Deposit authorization should be completed by the Claimant and returned to CNA. On receipt and verification, CNA will set up the Direct debit payments to the bank accounts provided by the Claimant in the Form. Direct Deposit is only available if your financial institution is part of the New York State ClearingHouse. The depositor’s name must appear on the account.
Alternatively, a copy of the form can be obtained, by request, from the CNA Claim Professional who established the compensation payment as part of the workers compensation insurance claim.
